Difficulty Level wouldn't change, no. The difficulty here is skill-based. Not Damage-Based. Essentially it just means hyper-aggressive AI. Damage isn't reduced or enhanced. whatever Naruto dishes out is what Madara is going to take and vice-versa. Not accounting for resistances. Madara is weaker to taijutsu than ninjutsu, so he would take more damage from the former than he would from the latter. But he would still take considerable damage if Naruto was leveled up all the way.
