while it's not alternate history, the novel 'The Pleasure Lord' is a good one. medieval setting, kingdom building, reincarnation, the biggest difference is that it's a cultivation novel with all of the stuff that comes with. although it does actually have some uniqueness. although I haven't read every novel, so don't quote me on that.
