You know some people might not like this novel as it isn't really something that grabs a lot of your attention all the time, but I enjoy the fact that it's calm tone and pacing is different from most of hyper action I read.

I hope his first loves disgusting change in personality actually ends up making sense. As for the sister it seems she let her revenge warp her and made him an enemy as an obstacle of her taking over the family to force them to help her fight for her revenge. Spreading rumors about him and helping damage his relationship further with his first love made his reputation horrible despite not doing anything to deserve it. Causing him to lose his position in the family too and then when he finally snaps and wants to retaliate the hero swoops in and kills the villain. Though that dream if it was actually canon and not just a dream alters the story of where he dies with the ring and love note on him. So I'm going to assume the dream was a dream.
