It's a constant state of fear not knowing what could happen next and having no one to turn to.
The silence. Having no one to talk to, no voices, just the sound of the wind or your own footsteps. It can drive you insane.
For me, it would be the slow realization that there's no hope for the future. You're the last person, so there's no chance of rebuilding society, no chance of having a family or any kind of human connection again. And as you wander through the empty cities, seeing all the things that were once symbols of human life, like schools, hospitals, and movie theaters, now all empty and decaying. That sense of total and final loneliness is the most terrifying part.
