You never brought up what happened with Tengen and Riko. Since Toji kept raising his kids, he didn't take the hot on her, so Riko should have survived. But that mission was never brought up since it was supposed to have happened shortly before Geto burns down the village.

I think I figured out how Megumi can beat Mahoraga, looking back, it was kinda obvious. While both Naohiro and Mahoraga require their wheel to turn, only Mahoraga has an external and physical one. If Megumi can break Mahoraga's wheel before it gets the chance to turn, then he should be able to deal with it faster. Mahoraga is most dangerous in a prolonged fight, so wearing it down isn't an option unless he can stop it from adapting in the first place.
