I think it might be because the 18+ chapter where sakuya become a futanari, that count as gender bender. However, I suggest you put futanari as the tag. Gender bender usually consist man to woman or the other way around, even if the term related to all kind of changing someone's gender. that is my opinion, you are free to deem the tag as you like and see.
