Kakashi went into coma until Tsunade came and helped him. He may be still in a coma if Tsunade never came.

I don't know. I read that Itachi's exam was the same as Naruto's. It may be a bit different like the 10th question in Naruto which Ibiki added But, it's mostly the same.
