Technically, it is the player who uses the "Throw voice" shout in skyrim. At least I have never encountered any NPC use it against me.

Just think of it as a mental sound effect. Like for example you receive terrible news. You won't hear any sound effect but the abrupt change of mood and emotions make you feel like your heart heard something thump down hard. Like when you discover something you can't accept about someone you like, there is no actual sound of glass shattering but you feel it in your mind.
