They weren't appointed by people or by voting. Every single Mayor was chosen by the higher-ups of the body governing the whole state.
Lilly wasn't really informative in this area as there were various governments in this world. The most common one was Royalty, where a single family or clan governed the whole state on the basis of strength.
Lilly wondered what type of government the state of Corenta had. But she was clear about one thing. Even if the Mayor was appointed by the Royalty, it didn't matter as they had to follow certain laws.
Laws that her father had set. Nobody could go against them.
The Law being talked about here stated that a person in power or, more like, in government cannot flee upon any crises, leaving their area of governance to death.
