Start of an adventure

ya know, I never really thought of an afterlife, but I didn't expect a weird hospital waiting room.I am, or was a 26 year old guy who apparently can only date crazy girls. I was getting off work when my ex pulled up and kinda shot me as I walked out the doors, I remember making eye contact and seeing her cry and then nothing. When I came to, I was in my work clothes just sitting in this waiting room, but I had a black band on my wrist that had the fallout vault symbol on it. I remember reading fanfics of this happening but I couldn't believe it, it's fucking real!! my name was called and a really pretty and nice looking nurse smiled and waved me over. she asked to see my wristband and she stopped smiling and asked if I knew what I was getting into. I smiled and said yes, I'm gonna change fictional history and make things better. she smiled again and turned and told me to follow, she led me to a room that was so white that I had to squint to see the table and chair and grumpy looking man. He had stubble and looked as if he had been awake plenty more than 24 hours. He waved me to sit and asked me for my name age and the date I remember. I told him everything I remember and he shrugged when I finished and said that didn't matter anymore. I had 4 choices and I had to decide if my heart and head really desired what I wanted. There was Washington, Nevada, Boston, and west Virginia, but I knew without a doubt what I wanted and how I wanted to fix everything. I smiled and told him what I wanted. To be exactly like the game, pip boy, experience, levels, but I had concerns about dying again. If it would really be the same as the game. He laughed and said I'll get 5 chances, he went on to say that I could get mortally wounded or completely killed and I'd wake up at the last place I fell asleep at.
