It doesn't stop ALL emotions, he is not a cold unfeelingmachine. It stops only emotions if those are sufficiently powerful and reach the threshold where they would get suppressed.

Great Red did force Ophis out of the Dimensional Gap and made her look out for people to force him back out. So Ophis lost her battle with him, so she is weaker than him.
But Ophis was weak. For a lesser race, perhaps, she was incredibly strong – but she was weaker than the Great Red, that was why he could her off the Dimensional Gap. She only wished to exist in the void – but she did not realize that by doing so, her existence would not last.

I would think that most of the playable characters would be stuck with IV that blocks power below level 80, and V would be a prerogative of bosses and some exceptional players like those with World classes that blocks below level 90

He can't get skills, only spells, as was stated

HP stat and HP amount are different things
In other words, it was much more effective for low-level players, whose HP might only be a hundred or two hundred, but for high-level players it was useless garbage. Even throwing such items in the trash would not bother any of them, they would not bother to clutter their Inventory with such low-level items. No one except Momonga himself, of course.
