This woman doctor is rather bold. Isn't he her senior? Who does she think she is that she can lecture him (and rebuke him) on his decision and behavior? As Evelyn said, he is not the only doctor there and he even told her which doctor to go to. He may be the new head of the department, but he doesn't have to answer to the whims of a wealthy patient. If he was that person's primary doctor, they would more than likely have his personal cell number and would have contacted him directly.

Obviously the neighbors don't talk to each other because the other guy was complaining about Sawyer breaking all of his mirrors and Sawyer walked through the front door of that house.

The "one who is learning about this with the witch" is Hailey. If Ruelle had looked for and discovered it completely alone maybe would have averted this consequence, but Hailey is her helpful friend.
