no, this is coherent enough, there are some fics that need a rewrite but this is not one of them. All you need is regular updates and future planning, so you don't write redundant chapters.

this is really setup dialogue ,no-one really says that if they wanna leave.If you don't like therapy just make some menial conversation and point out how it doesn't help at the END.

therapy isn't really all that good for most people, people have been dealing with life for a very long time and they did it well. Therapy can work great for some people but it's mostly an asthetic now. People think going into therapy will solve their problems or makes them better ,when its mostly someone who doesn't care about you getting paid hourly to make sure you keep coming back. Your first choice should always be yourself and self-reflection really works.
