I want some surprise, as this is everything going in mc's favour. A major plot point i see that could be it is Patel's death. heart is an organ which is unpredictable, healthy living can after having small heart alterations posses no guarantee that you would live long. It might in cases hasten the heart disease. A major plot point would be if instead of making patel's health better, the changes he takes for his healt hastens his death. this could both give mc a major problem he would have to overcome, gives area for antagonists to come (politicians wrestling to take over patel's things, such as RAW, given its under the prime minister's purview) and also comes for great character development as vikram then would have to take greater responsibilites,
