Chunin can make 3 but it is not used in combat because it will drain their chakra so much it will become an hindrance that's the reason only Naruto can do it when he is a training for rasen shuriken.

Lol, No actually that is how Japanese people introduce them self it is still present to this day. They introduce their Surname or family name first then they said their full name so that's why japanese always said Family name-san or something.
