Hiruzen greatest fault was that he was too trusting, to Danzo, to Orochimaru. his failure to keep them in check had continously allowed Danzo to act without limit, even when he disbanded Root he never bothered to Monitor or keep a close eye on Danzo to ensure he had completely obeyed. he allowed a private army to grow beyond the oversight of his own regimen simply because he trusted his 'friend' too much. The uchiha matter could have been avoided if Shisui simply use his mangekyo, but no Danzo interference has completely severed that option leading to the bloodbath.
