He only picked Naruto world as his first world to get stronger. He didnt feel the need to change anything because he was eventually going to leave. He could stop the uchiha massacre because he would either run into Itachi who he couldn’t beat or Obito who would gladly kill him.

At some point he’ll defect because he won’t have anything to gain from staying in konoha anymore. Itachi uses the eye to free himself from the Reanimation jutsu in future and I don’t want to mess with that part.

He doesnt want to draw attention from danzo yet. And danzo only has one of Shisui’ eyes.
