Massaging from the beginning wouldn’t cause a PE or Stroke. It’s preventative care. It’s only advised not to if the patient has a condition affecting their coagulation or smth like DVT. If Nick disregarded Horua for days on end and then started moving his limbs, then possibly. But if he moved his limbs on a regular, daily basis (like described), then it’s preventative care.
Main point is that you are supposed to massage because if you don’t, a clot will form. It’s completely different if the patient has DVT, then sure, massaging could release the clot, but that’s not happening in this situation
Muscle movements and gravity are the main determinants in blood flow for veins since they do not have the same amount of muscle in their walls when compared to arteries. Thus massaging extremities not only reduces the chances of muscle wasting, but also the chances of a thrombus forming, it detaching and becoming an embolus, an said embolus travelling to the lungs, and the patient dying from a pulmonary embolism and/or stroke.
