In a lightnovel, Tsuki ga Michibiku Isekai Douchuu, the MC meets one of the founders of the world's 'adventurer's guild' and he explains that the reason why he made the guild is specifically so that those who take too many risks to climb the ranks die quickly. People can still rank up reasonably though. But even though people are dying left and right, a lot of the guys who join the guild are oblivious to it and keep doing dangerous and risky stuff for riches anyway. Maybe, eventually, enough dumb people die that they learn to become decent people.

The people in this world didn't really have any particular or distinct accents, and considering that English was the only language in this world, it stood to reason that second-language accents simply wouldn't exist.
