If they are already saints and receive no other upgrade, they would miss the biggest reward. The Spell may be terrible but it is always fair. There should be some sort of balance.

I think they will also find a way to transcend as well. Otherwise their corrupted selves make no sense. I do not think corrupted creatures can increase their rank, only their class. If I am right, this means that they will emerge as sovereigns after conquering the nightmare. I am not sure though, as that may accelerate the story too much.

If you are talking about brain tumor, there is certain unfortunate statistics. It is more likely in adolescents and young adults than middle age people.
