there is no openings from normal human viewpoint. all of his guards are normal. You might expect the unexpected from SCP object but you can't be prepared from them all. how many SCP object are there? countless. each with different ability. there will always be loophole when you face the unexpected. there's always first time. it's their job to prevent the second time. Let's just be honest, even the MC never thought somebody will phase through the wall. so, why be harsh to your guard who guard the door? he did his job. guarding the door. and no one barging in through the door under his watch.

no it's not. first, the room doesn't have surveillance camera. second, the intruder can phase through solid object. even the MC got surprised, it's normal for the normal guard even if he is class A to got one upped by the unexpected.
