Personally I agree with u but it didn't seem that capitalistic in the canon when it comes to hospital stuff XD... Someone has to teach them and they need to be paid even if the knowledge and materials are cheap...but they held a test, ideally that should be refundable at least partially, like the more u score in the test, the more of ur 'deposit' gets returned...XD if u waste the staff's time, u get nothing back...

nah, it's more straightforward than that. It comes basically after his name and titles. It's his thing, like Madara and Sharingan, Minato and speed/flashing XD/instant death, Tobirama and water... Naruto equivalent of your analogy would be basic laws/codes made by Hashirama upon finding/uniting Konoha ( like basic human rights and amendments in America XD) of which kids or new generation might not know/don't care of...
