They infact do count it starting at 1, as they count it by how many “calenders” you have lived through even if you were born at the start or end of the year, now as for ”most asian countries” those include China, Taiwan, both Koreas, Vietnam and Japan would be the nations I know who practiced this, atleast at somepoint with Japan stopping it officially a century ago and South Korea just starting to stop it…

Well, like I said, there can work, though it’s somewhat debatable if it could be considered as being used as an adverb, in which case “that” wouldn’t be applicable for the sentence

With “had had” its a bit tricky but typically in this case you would say “He’d had” instead of “He had had” as you would typically use the contracted form if possible in English
