I don't know if it's a plot hole but in ch 40 at the end mieko was learning 3 jutsus from the konoha library and one of the jutsus was a fire proof jutsu so couldn't hiruzen just use that to not get affected by his own flames in his fight against orochimaru?, just curious.

I think I saw something similar earlier and was hyped for it but it was deleted like at chapter 12 Hope you can take this further. Now I hope I can see the whole story.

Translation quality is good. And assuming that he keeps updating it will probably keep updating for a while considering that this is a translation. Story for now keeps flowing at a nice pace. Character can be considered a bit overwhelming in terms of growth.
