Just in case, it isn't Unohana or any named character, think of her as an original character, if you want. The significance of this scene lies in how the encounter influenced Kousei, to me her identity doesn't really matter that much.

Writing quality dropped massively as soon as you switched to first-person POV. What happened?
I'd finally one and I felt it the shift in my being. Evolution now that only one remained. The massive size I had as a Menos shrank as I became a hollow more fitting of me. As my soul shaped my forn to match the image that best describes me.
