26 Double Care - 1d Medical

Niall walked back towards the living room. Another TV show was playing as he ruffled through the hair of his 13 year old brother Sam. Niall was full time caregiver of Sam since the dead of their parents. He also had full parenting over Sam's twin sister Sienna. Niall got full parenting over Sam and Sienna after their parents died in a plane crash 6 years ago.

Sam, who was currently watching Bumba, sat concentrated on the coach. It wasn't odd for Niall that Sam was watching it since he had down syndrome and autism with it. His mental state and social state would never be higher then that one of a three year old. Luckily Niall wasn't the only one on this case since his fiance Harry was just as helping and treated Sam and Sienna as if it were its own children.

"Sam could you maybe help me for a sec," Niall asked as he was standing on a chair. Sam nodded and stood up from the couch. "You hold this and I'll get some things to decorate." Niall handed Sam the other end of the party garland and hopped of the chair. "Ni back soon?" Sam asked his oldest brother who nodded. On the party garland stood 'welcome home Sienna'.

Sienna had been ill for a long time. Not just the flue but really ill. Sienna was 10 when they found a brain tumor by Sienna.

"I don't trust it anymore Haz," Niall spoke to his boyfriend Harry. Sienna slept in the middle of their bed because she had woken up once again with an tremendous headache and had thrown up many times that night. It was already for like a month that Sienna felt that way." We'll see Ni," Harry spoke as he tangled his fingers in Sienna her hair. Suddenly Sienna her eyes shot open and Niall quickly sat up. "Sienna what's wrong girl," Niall softly spoke. Sienna quickly covered her mouth and Harry quickly grabbed the bucket next to his side of the bed. Just when Harry put the bucket under Sienna her chin she began to threw up violently. Niall comforted his sister and got tears welling up in his eyes. "You finished?" Harry asked as Sienna began dry heaving above the bucket. "Yes I think I am," she murmbels out. Harry found it odd that her speech was just stottering because Sienna was quite a good speaker. Niall didn't seem to notice and got out of bed. "Alright Sienna let's clean you up," Niall said as he pulled the duvet from Sienna. As she wanted to get out of bed she realised that she didn't feel her left leg anymore. "Sienna what are you waiting for," Niall asked as he waited for Sienna to get out of bed. "I-I can't feel my leg Ni," she said crying. Harry immediately put on his shoes and picked Sienna out of the bed. "I'm getting her to the hospital Niall. You stay here because you can't probably drive at this moment." Niall nodded while he cried as he kissed his sisters head. Harry went downstairs and covers Sienna with one of his jackets. He made sure that his cellphone was in his pocket and that he had the housekeys before he left with Sienna to his Rover.

Sienna cried in Harry's neck who put her on the front seat of his car. "You'll be alright little one," Harry reassured her. "Haz I don't feel good," Sienna softly spoke. Just when Harry wanted to ask further Sienna got a seizure. Harry luckily pulled up the hospital driveway. He unbuckeld Sienna her seat belt and ran with a still seizing Sienna in his arms inside the hospital.

"Please help me," Harry yelled inside the empty hospital, which wasn't odd since it was only 4.15 am. Suddenly a young looking male came running down the hallways who heard the commotion going on at the front desk. "What's wrong," the guy asked as he found Harry crying holding a seizing Sienna in his arms. "I don't know," Harry cried. The guy nodded understanding from Harry's reaction that she didn't have a history of Epilepsy. "You oke to carry her of do you need a gurney?" the guy asked. "I'll carry her." The guy made a gesture that Harry should follow him which Harry did.

"Alright just call me Liam," Liam said to Harry. "I'm Harry Styles, her name is Sienna Horan and she's ten years."

Liam nodded and scribbled things down. Sienna stopped seizing and Liam pulled the girl into the recovery position. "What are her symptoms?" Liam wanted to know to get the picture complete. "Ehh yeah she has tremendous headaches and she keeps throwing up," Harry started, "this night she kept throwing up and crying about her headache so I let her sleep between me and her brother so that we could act quickly if she went down. Suddenly she began to throw up again and than she said that she didn't have a feeling in her left leg anymore." Liam looked with worried eyes towards Harry and pressed a number on his pager.

"Did her speaking change?" Liam wanted to know. Harry sadly nodded and explained that her speech was more stuttering and it took Sienna very long to find the right words.

"Alright I'm whe-," before Liam could end his sentence he heard vomit hitting the floor. He quickly put a spit bucket under Sienna her chin who was still unconscious. "It's alright girl. You're going to be fine alright," Liam softly spoke. After a few minutes Sienna was done vomiting but still hadn't regained consciousness. Liam thought it was best for her not to try and get her out of the state because the things Liam was about to preform could be pretty traumatic for the young girl. "So what I was trying to say," Liam said again towards Harry, "Sienna will need a CT in order to find out what's wrong." Harry nodded and let Liam take Sienna towards the CT scan.

After 20 minutes Sienna got back. She was somewhat awake and had already an IV. "Alright Harry," Liam started as he crossed his arms, "I'm going to discuss together with an colleague. We also got some blood samples of Sienna. If something happens just push this button then I'll be coming as quick as possible." Harry nodded while Liam left for an awful discussion.

"Alright Lou," Liam said as he took a seat next to his love and colleague. Liam was a pediatrician while Louis was a neurologist so it happened not much that both of them worked on the same case. "Alright Li," Louis sighed and he pulled up the pictures of Sienna's CT, "she clearly has a medulloblastomas." Liam threw his pen away frustrated. He worked for ten years as pediatrician and never had to diagnose a child with a medulloblastoma. "Telling from the CT and the symptoms you gave its blocking her CSF which is quite operational and it looks like that I can cure it with chemo as well. It's too large to operate at once so my suggestion is to operate tomorrow and take out what I can take out and then immediately start chemo therapy." Liam sighed and nodded knowing his husband was right." Hey Li she'll get through this alright. We're going to monitor her everyday. I know your feeling, I felt the same when I got my first cancerous case." Liam wanted to react but his pager went off.

"Fuck it's Sienna," Liam said as he threw his chair back and ran down the hallways with Louis closely following behind him. Liam shot inside the cubicle and saw how Sienna was seizing again. He sadly sighed as Louis also made his entrance. "Doctor Tomlinson could you take Harry with you. I can handle it on my own so you can explain everything." Louis nodded and walked with Harry towards an empty exam room where he explained everything to him.

Liam was busy keeping Sienna alive as she stopped seizing. He slowly rolled her back because he saw that she was waking up." Hey little one," Liam exclaimed as he saw that Sienna was waking up. "What's wrong with me Liam," Sienna silently cried. Liam took a deep breath. "Alright Sienna I'm going to be fair to you," Liam started, "there's something growing in your brain that isn't supposed to be there. With a difficult word it's a medulloblastoma, but it's just a brain tumor. There's another doctor talking to your dad -," Sian a interrupted Liam. "Harry isn't my dad. He's the boyfriend of my brother Niall. I have a twin as well he's called Sam only Sam likes to play with dolls because Sam has down syndrome and autism but that doesn't matter because I still love him. My parents died however on a plane crash 3 months ago. Niall first thought that grief got to me and that was why I had been so ill."

Liam sighed sadly and filled it in on his chart.

" Well Sienna it lays a little more complicated," Liam said as he sat down on the bed, "look we're going to operate you tomorrow. Than I'll get you chemo. You're going to get bald soon but your beautiful hair will grow back. Also you can go home sometimes because when your on the chemo you have to stay one week."

Liam stood up and walked towards the closet and grabbed a robe and a box of letters.

" Here," Liam said while he handed Sienna the box, " I'm going to give you the robe so you can search for your letter of your name and than I'll help you put it on the cord."

Sienna carefully picked her letters S-I-E-N-N-A.

"Done," she proudly said. Laim explained to Sienna that every time she got here she could get a new charm to put on the cord and for an operation there were 10 charms to give away. Sienna looked in the box. "Chose 10 now because maybe we'll forget them by tomorrow," Liam said as he nodded the robe in order not to let Sienna her name fall off.

Suddenly Harry came back and cried. He pulled Sienna close to him and cried together.

Sam was still holding the party garland. Niall came back with his pins. "Here Samie," Niall said as he handed his brother the stuff to pin the garland to the wall. "Sienna coming home?" Sam wanted to know. Niall gratefully nodded and hugged Sam.

"Yep Sienna is now clear of monsters. Because uncle Lou and uncle Li scared them away."

Sam didn't know what was wrong with Sienna and didn't seem to get how a tumor worked, Niall and Harry decided to tell him that there were some monsters inside Sienna and that the doctors were scaring them away. Sam understood and always came to Sienna wearing a mask to scare the monsters. He made it all by himself and Sienna always had to laugh.

After a few chemos Sienna began to lose her hair and asked Sam if she could shave her head. Sam loved every second of it while Sienna had more difficulty knowing that she would be bald. She however refused to get a wig since Sam was very proud at his creation. He was proud of his bald sister.

Sienna was ill most times and very tired but the fun between the two stayed. When Sienna felt well enough she watched a film with Sam he liked. Sienna didn't care how childish it was as long as her twin was happy she was as well.

Sienna had to go Liam and Louis a lot so they both began to see the two doctors as their uncles.

Sian a was now cleared of cancer today and she was allowed to go home, Liam and Louis would be coming over tomorrow and promised the twins to go swimming. Sam was already looking forward to it but he looked most forward towards his sis coming home and giving her the drawing and the cake he baked.

Sam sat by the window waiting for Harry's range to arrive. When Harry's car eventually pulled up the drive way, Sam sprinted towards the door to open it for his sister.

"Sienna," Sam yelled as he wrapped himself around her leg. Sienna laughed and cuddled with Sam. "Het bro you happy?" Sienna asked. Sam gratefully nodded and explained that he was happy that uncle Li and uncle Lou had scared away those monsters.

Harry also made his entrance and walked straight towards Niall.

"Ehh Ni there's something Louis told me before I left," Harry said scratching his head, "for tomorrow, can you go with Louis swimming because Liam suddenly has to work and I need to work as well." Niall nodded but knew there was more information coming.

"And it's Sienna. Louis told me that she could still undergo the effect of the tumor which will be shown as seizures."

Niall nodded and now knew that Sienna needed to be controlled every day and had to be on medication as well.

"We'll get through," Harry softly whispered as he kissed Nialls head.

"You ready Sienna," Sam whined as he was already done. Sam waited outside the changing rooms of the pool on his sister. "Yeah Yeah almost young man," Sienna spoke soflty. She looked at her stomach which was fully flat, she could count her ribs, it didn't look right and not pretty either.

Sam knocked on the door. "Yeah Yeah Sam don't worry mate," Sienna laughed as she stepped out of the changing room. She put her clothes in the cabinet and walked with Sam towards Louis and Niall.

Together they walked towards a spot that Niall had chosen and before they even sat down, Sam was already in the pool. Niall came after Sam, who did learn how to swim amazingly, but who wasn't the strongest swimmer out there.

"Sienna I need to discus a few things with you alright," Louis started as Sienna sat down, "if you feel tired or off then just come to me." Sienna nodded knowing what Louis meant since she was just clearer of a braintumor.

"Alright than let's go," Louis said as he grabbed her hand. Sam was already swimming with Niall while Sienna and Louis chose to go from the slides.

It was a long day of swimming and suddenly Sam noticed the diving spot. "Sienna there," he pointed to the diving platforms, "wow."

Sienna laughed and walked with her brother towards the platforms. Louis and Niall weren't a great fan of that idea but Sienna promised them she would only jump once to make Sam happy. Eventually the two gave in but insisted of staying close to the platforms if something went wrong.

Sam was the first one to jump from the platform. Sienna heard Sam giggle as he came back up. She suddenly began to feel weird but she just let it pass by and would tell Louis after she jumped. Sam was down yelling for his sister to come down as well.

Sienna looked over the edge and jumped. When her body however hit the water surface Sienna stiffed and was about to have yet another seizure.

Sam didn't notice but he thought it took a while for Sienna to get back so he decided to tell Louis.

"Uncle Lou," Sam whined. Louis looked up from Niall since they were in a informational conversation.

"What's up Sammie boy," Louis asked as he crunched down to get on eye level with the boy who was still in the water. "Sienna no up," Sam explained but Louis had no idea what the boy meant. It was until Sam pointed to the water that Louis understood that Sienna never came back to the surface. Immediately Louis scanned the water and yelled for Niall.

"What's wrong," Niall asked. Sam explained once again and Niall looked directly at the surface as well. Louis couldn't find Sienna and neither did Niall and their worry began to grow.

Suddenly Niall saw something at the bottom that looked a lot like her.

" Found her," Niall yelled as he jumped inside the pool since he was the strongest swimmer of the two.

"Sam I think you need to go to my bag and get my phone. Would you do that for me?" Louis asked Sam not wanting the boy to see what was going to happen next.

Niall came back to the surface with an unconscious Sienna. He laid her at the edge of the pool and Louis immediately checked for a pulse.

"Get a lifeguard Ni, " Louis yelled as he began cpr since Sienna didn't have a heartbeat and wasn't breathing either. Some members of the public saw what was going on and cleared the whole area so Louis wasn't stopped by people who were interested. Louis hated that people and loved the ones that actually made sure that everyone left.

Louis saw in the corner of his eye Niall rimming back with three lifeguards. All three of them were shocked but ran towards Louis nonetheless.

Louis stepped aside because he wasn't able to resuscitate Sienna because he was just way to tired to do it correct. Niall, who ran into Sam and got the phone from San, gave Louis his phone as he slowly slid down the wall. Louis on the other hand phoned the emergency services and told them that there had been a drowning in the pool and gave them the Adres.

After they hung up, Louis got back to the Boys who were still reanimating Sienna. Louis saw that the boy who needed to give chest compressions was beginning to tire out.

"I'll take back over while you wait for the ambulance outside," Louis ordered the boy. The boy nodded and ran towards the emergency exit while Louis continued the chest compressions by Sienna.

Three minutes later a whole team of paremedics came running down the poolside. It wasn't until Niall noticed the last person that came running in that the person knew who was laying there. Liam got called up for this case since there was a child involved and he hadn't had a case back in the hospital. "Sienna?" Liam softly asked. Niall cried and slowly nodded his head. Liam ran towards the scene where he found his husband resuscitating Sienna. "I'll take over Lou," Liam softly spoke as he crunched down next to his husband, "go get me towels to dry her so I can use the defib."

Louis nodded and ran past Niall who clearly didn't have his emotions straight anymore but Louis couldn't blame him. Liam and Louis were trained to save people under every circumstance, Niall just wasn't and Louis didn't blame him for showing his emotions.

Liam continued to do chest compressions however he was much stronger than Louis and the boy so after a few great pumps he heard a loud crack. Liam just broke one of Sienna's ribs and immediately knew that he should be a bit softer.

"Alright I want 20cc of adraline," Liam ordered the female paremedic. She set up a shot and gave Sienna the dose of adraline Liam requested.

"Li here," Louis said as he passed Liam the towels. He immediately began drying of her chest and laid the pads of the defib on her chest.

The other paremedic began to charge the pedals to 200 volts. "Clear," Liam yelled to everyone once he placed the pedals on the pads. He shocked Sienna with 200 volt but didn't have an effect. "Charge to 360," he yelled again. The paremedic did what Liam asked for and charged it to 360. "Clear." Liam shocked now 360 volt into Sienna her system. Suddenly Liam heard the familiar sound of a heartbeat. "Alright we got her back," Liam sighed, "intubate and make clear for transport." The two paremedics nodded and intubated Sienna. Liam walked towards Niall.

"Ni hoe did it happen?" Niall shrugged his shoulders. "Don't know," he simply replied, "Sam told Louis that Sienna hadn't came back up when she dove. I found her at the bottom of the pool."

Liam could fill in himself what happend, Sienna most likely got a seizure when she hit the water surface and slowly had sunk to the bottom.

"Docoter Payne we're ready," one of the paremedics yelled. "I'll meet you in the hospital," Liam said as he patted Niall's leg.

"Alright. 13 year old Sienna Horan, has a history of a medulloblastomas. Drowned in a pool due to a seizure. BP 70/35. Hasn't regained consciousness and isn't able to breath on her own. Had 20cc of adraline at the scene and broke a rib due to chest compressions," Liam said as he handed over Sienna to the nurses. Liam quickly gave his jacket to the paremedics and began to connect Sienna on a breathing machine. Liam rubbed Sienna her sternum to wake her with no luck." Doenst matter love I know you're trying."

Liam sat down and filled in Sienna's chart further. Liam also made sure that he would arrange a meeting with Harry, Niall and Louis in order to prevent these things from happening again and setting up the right treatment for her seizures. Liam felt sorry for the girl who almost passed away under his watch for the second time.

Suddenly Sienna her eyes shot open and she looked scared. She immediately began to scratch the tube in her throat. "Sienna wait let me help," Liam said as he shot up from his chair.

"Three big breaths alright then it will be over."

Liam pulled the tube slowly out of Sienna her throat. She gagged when Liam passed her gag reflex. Not much later Liam successfully removed the tube and got a mask that would provide the oxygen.

"Than-," Sienna couldn't finish her sentence because she began to throw up violently. Liam quickly shot to action and got a spit bucket. It was all water that came out of her system.

Secundair drowning Liam thought while he rubbed Sienna's back.

"We're not making a habit out of this my young lady," Liam laughed and even Sienna smiled. "I'm going to fill in your uncle and Nu so I'll be right back."

"Uncle Li wait," Sienna soflty spoke, "I love you." Liam smiled and placed a kiss on her forehead. "I love you too my beautiful Sienna."

Liam walked into the family waiting room and found his husband and best friend waiting. "How is she," Niall immediately asked as he saw Liam entering the waiting room.

"She's stable," Liam sighed, "she had a seizure when she hit the water surface so that's why she sunk to the bottom. I accidentally broke one of her ribs while giving cpr but within a month she'll recover since it a small and clean break. The thing that I'm worried about most is that she just threw up a great amount of water. So I need to keep her here for 48 hours to monitor her inorder for her not to die of something we call secundaire drowning." Louis and Niall understood and wanted to see Sienna.

Once Niall entered the cubicle he ran into Sienna her arms.

" Never ever scare me like that again," he soflty spoka as he hugged Sienna.
